If you or a loved one need help navigating the mental health care system,  or you need geriatric care help for a parent or other elder relative, Geisel Advocacy for Patients LLC can reduce your stress that results from wading into a complex and unfamiliar arena. GAP, based in Northern Virginia, offers health care navigation  services throughout the Washington, D.C. metropolitan area. We make sure that you understand the diagnosis, treatment options and the questions to ask, so that you can make informed decisions. Additionally, Geisel Advocacy for Patients strives to promote understanding of the health care system and medical specialties in order to make appropriate care accessible. I am open to patients needing the services listed below, many of which can be provided throughout the United States via telephone and technology.


Following is a list of services that Geisel Advocacy for Patients LLC offers in mental health care and geriatric care:

Research diagnoses, medications, treatment options and providers.

Facilitate shared decision-making among individuals and their families or other significant parties.

Help patients gain comfort in communicating with and directing questions to health care providers.

Coordinate care so that every provider treating the patient has complete medical information from other specialists on the care team.

Attend medical appointments when requested, necessary and appropriate.

Assist and monitor patients during hospital stays to ensure all needs are met and necessary information flows between providers and family or other people designated by the patient.


Process

When you call or email Geisel Advocacy for Patients LLC, the first step will be to have a free phone call lasting approximately 20 minutes to determine whether your needs  for mental health or geriatric care support are a good fit with the philosophy and services of Geisel Advocacy for Patients LLC.

If we decide to take the next step, we will schedule a meeting, lasting about one hour, to discuss the medical needs and concerns in detail and complete the contract. Based on that meeting, Geisel Advocacy for Patients LLC will develop a care plan that lays out what will be done for the client.

Once the client approves the care plan, work will begin.    

Fees

The initial meeting, whether in person or via telephone or an online audiovisual service, and the written care plan, are billed together for a total of $150. Services detailed in the care plan will be billed at an hourly rate ranging from $75-$90.
